About the role

Care Navigation and Benefits Specialist - Remote
If you know the VA system well enough that colleagues have turned to you when a Veteran's situation became complicated, this opportunity may be worth a conversation.
We're looking for an experienced care navigation professional who can bring that knowledge to a growing Veteran-focused care coordination program. You'll help Veterans navigate healthcare, benefits, behavioral health services, community resources, and other support while also helping the broader care team handle complex cases more effectively.
What makes this position particularly interesting is the opportunity to do more than manage individual cases. You'll serve as a resource to other care coordinators, help improve Veteran-specific workflows, identify gaps in the program, contribute to training, and help shape how care coordination is delivered as the program develops.
The Experience That Matters Most
We're particularly interested in someone whose background brings together Veteran or VA experience, care coordination, and increasing program responsibility.
You'll need:

5+ years of experience as a clinical social worker, care coordinator, care navigator, or in a similar field, with demonstrated progressive program management experience

Direct experience working with Veterans and/or within the VA system

1+ years of experience working successfully in a remote environment

Knowledge of VA healthcare, Veteran benefits and resources, behavioral health, and social determinants of health

Experience supporting medically and/or behaviorally complex populations

Experience documenting cases within a care management or electronic health record system

Strong communication, collaboration, and problem-solving skills

Experience with motivational interviewing is also important.
A social work license is not required, although candidates with credentials such as an LMSW or similar professional background are encouraged to apply. Fluent Spanish is helpful but is not required.
How You'll Make an Impact
You'll help Veterans understand and navigate available resources, including VA healthcare, benefits, community care, and other services that address healthcare and social needs.
You'll also guide complex Veteran cases, help determine appropriate referrals and escalations, and step in with direct Veteran support when specialized assistance is needed.
Beyond individual cases, you'll help develop Veteran-specific care coordination workflows, identify opportunities to improve the program, and work with care coordination and training leadership to strengthen the team's understanding of VA healthcare, benefits navigation, Veteran resources, and common barriers to care.
In short, your experience will help both the Veteran in front of you and the team supporting the Veterans who come next.
Location, Schedule and Compensation
This is a full-time, 40-hour-per-week remote position working Monday through Friday during normal Central Time business hours.
Candidates must reside in the Central Time Zone in Texas, Alabama, Mississippi, Louisiana, or Tennessee and have a dedicated home-office environment.
The anticipated salary range is $80,000 to $100,000, with potential flexibility based on the selected candidate's experience and background.
Travel is not required.
